Non-members are welcome to participate in a variety of activities year round including private swim/tennis lessons and our Camp Miranda summer program. This form is required before non-members can register for any offerings. Please fill out the form below and await further instructions.

AUTOMATIC PAYMENT POLICIES:

Foothills will send Electronic Statements to the email address provided on the 1st day of the month. The Electronic Statements are for your information ONLY. All charges on your Electronic Statement are for participation in programs in the month prior. You have 9 days to report discrepancies on statement before payment is withdrawn. Foothills will automatically withdraw the ‘Balance Due’ amount from your Bank Account on or about the 10th day of the month.

AUTOMATIC PAYMENT AUTHORIZATION:

By signing below, I am agreeing that I am either the account holder or have the authority of the accountholder to authorize Foothills to automatically withdraw from above named account. I am authorizing Foothills to make a onetime debit of $10 to authorize my bank account (this amount will be applied to your account as a credit). I am also authorizing Foothills Tennis and Swimming Club to direct debit the total amount for all charges incurred, by any family member, to my bank account provided herein. I understand that to receive monthly statements of these charges before direct debit on or about the 10th day of the month, I must provide a current e-mail address. I agree that I will pay for the programs and/or activities in accordance with the issuing bank direct debit agreement

NON-MEMBER POLICIES:

If for any reason you need to cancel your lesson you must cancel at least 7 days before the lesson date, or you may be charged a 50% cancellation fee. No-shows and cancellations within 48 hours of the lesson may be billed for the full amount. Cancellations are to be done through the Lesson Administrator, please email [email protected], if less than 24 hours please call (650) 493-8540 ext. 0 as soon as possible. Non-member participants may arrive up to 5 minutes early and must leave the grounds promptly after.

NON-MEMBER WAIVER & RELEASE AGREEMENT FOR ADULTS AND MINORS:

The undersigned understands and agrees that while on the premises of Foothills Tennis & Swimming Club (FTSC), whether using the exercise equipment, participating in any programs or events offered by the club or making general use of the facilities, he or she is doing so at his or her own risk. If the undersigned is the parent or legal guardian of a minor entering the FTSC facility, he or she expressly acknowledges that he or she is relinquishing, on behalf of the minor child(ren) identified above, all rights the child(ren) may have to sue FTSC for injuries arising out of the minor’s use of or presence upon the FTSC premises regardless of whether the injuries were caused by the negligence of FTSC, its employees or agents. FTSC shall not be liable for any damages arising from personal injuries or property damage sustained in, on or about the premises of FTSC, specifically including, but not limited to, all club facilities (indoors and outdoors), the pool area, all recreation areas, all exercise areas, all play areas, the clubhouse and the parking lot. The undersigned assumes full responsibility for any injuries or damages which may occur while on the FTSC premises, regardless of whether such injuries or damages are caused by the negligence of FTSC or any of its employees or agents. The guest also expressly acknowledges that by signing below he or she is relinquishing (on their own behalf and on behalf of all minor children listed herein) all rights he or she may have to sue FTSC for injuries arising out of the use of its facilities or its services. This release is intended to be interpreted as broadly as allowed under California law.
